Please note: All paperwork & questionnaires in Charm must be completed 24 hours prior to your initial visit. If they aren’t completed prior, we will have to reschedule.

After scheduling, you should get an email about setting up your Charm portal.

Charm is where you can message your provider, view your medical records and request follow-up appointments.

The cost of the appointment will be charged at the time of your appointment. We also can provide superbills at that time.
